Anotace knihy

This comprehensive survey focuses on African healthcare and institutions from the pre-colonial era to the present time. From the late 19th century onward, healthcare policy in Africa has been firmly embedded in historical, socio-economic, political, even cultural processes that can be called the institutional environment. Colonialism, the Cold War, decolonization, the crisis of dependent capitalist development, the emergence of neo-liberal ideology, the global AIDS pandemic, democratization, and, finally, the reengagement of the African state and the "international community" in healthcare have all informed healthcare policy in Africa. The text uses a comparative approach to explain all such factors along with successes and failures in the delivery of healthcare in Africa. Further, it addresses the impact of globalization, Western ideas regarding disease, and international assistance and philanthropy on health policy choices, implementation, and results. In addition to the many case studies, the in-depth study of Botswana, Ghana, and Rwanda helps to exemplify the role of political institutions with regards to health policy in democratic, democratizing, and non-democratic states, which altogether are representative of the health institutions and policies across the continent. The book thus examines the local, national, and transnational constraints that have informed healthcare choices, such as elite preferences, limited budgets, manpower shortages, inadequate facilities, lack of drugs or equipment, and demonstrates the crucial role played by political systems and the institutional environment. It also shows that, although international assistance affects healthcare policy choices in Africa, it does not necessarily affect implementation and outcome. This exhaustive and groundbreaking text will fill a gap in the literature on healthcare and health systems and appeal to anyone studying African public policy, administration, and healthcare.
