A thorough review of the mechanisms and pathology of cerebral ischemic reperfusion injuries (CIRI) and their clinical implications. As more stroke patients have the clot that caused the ischemia removed within 6 hours of the event either through the use of medication or removal devices reperfusion injuries are growing in frequency and importance. These injuries include brain hemorrhage, brain swelling, and brain high-perfusion and can often be as devastating as the ischemic episode. It is crucial that the next step in stroke management is understanding this type of injury and developing effective strategies for mitigating the damage. Among the topics discussed are the mechanisms for early and delayed recanalization, collateral circulation, mechanisms of posterior cerebral circulation, spontaneous recanalization, and no or low reflow.
